সেবা গ্রহিতা যে সকল সেবা পাওয়ার অধিকার সংরক্ষণ করেন

১. স্বাস্থ্য উপকেন্দ্রে আগত নারী-পুরুষ, বৃদ্ধ-যুব-শিশু সকলকে প্রয়োজনীয় স্বাস্থ্য সেবা প্রদান করা হয়।

২. ডায়রিয়া রোগীদের জন্য ওআরএস সরবরাহ করা হয়।

৩. হাসপাতালে আগত প্রসূতি রোগীদের এন্টিনেটাল চেকআপসহ প্রয়োজনীয় উপদেশ দেয়া হয় এবং আয়রন

ট্যাবলেট সরবরাহ করা হয়।

৪. জাতীয় যক্ষ্মা ও কুষ্ঠ নিয়ন্ত্রণ কার্যক্রমেরআওতায় যক্ষ্মা রোগীদের কফ্ পরীক্ষার জন্য কফ সংগ্রহ করা হয়

এবং যক্ষ্মা ও কুষ্ঠ রোগীদের বিনামূল্যে ঔষধ সরবরাহ করা করা হয়।

৫. শিশু ও মহিলাদের ইপিআই কার্যক্রমেরআওতায় প্রতিষেধক টিকা দেওয়া হয়।

৬. উপস্বাস্থ্য কেন্দ্রে আগত রোগীদের স্বাস্থ্য, পুষ্টি ও প্রজনন স্বাস্থ্য শিক্ষা দেওয়া হয়।

৭. উপস্বাস্থ্য কেন্দ্রে আগত কিশোর-কিশোরী ও সক্ষম দম্পতিদের মধ্যে প্রজনন স্বাস্থ্য ও পরিবার পরিকল্পনা

কার্যক্রমপরিচালনা করা হয়।

৮. প্রয়োজনে রোগীকে উপজেলা হাসপাতালে রেফার করা হয়।

৯. আগত রোগী ও তাদের আত্মীয়স্বজনগণ স্বাস্থ্যসেবা সম্পর্কে প্রয়োজনীয় পরামর্শ ও উপদেশের জন্য সংশ্লিষ্ট

চিকিৎসকগণের সাথে সহজেই যোগাযোগ করতে পারেন।

১০. উপ-স্বাস্থ্যকেন্দ্রে প্রয়োজনীয় সংখ্যক নোটিশ বোর্ড সবার দৃষ্টি গোচর হয় এমন জায়গায় স্থাপিত আছে।

নোটিশ বোর্ডে প্রয়োজনীয় তথ্য লিপিবদ্ধ আছে।

১১. সরবরাহ সাপেক্ষে ঔষধসমূহ সেবাকেন্দ্র হতে বিনামূল্যে প্রদান করা হয়। তবে চিকিৎসার প্রয়োজনে কোন

কোন ঔষধ কেন্দ্রের বাহির হতে সেবা গ্রহিতাকেক্রয় করতে হতে পারে।

১২. বোর্ডে মজুদ ঔষধের তালিকা, প্রদানকৃত সেবাসমূহের তালিকা, সেবা প্রদানকারী চিকিৎসকের তালিকা

টানানো আছে।

Citizen Charter
The service recipient reserves the right to receive all services

1. Necessary health services are provided to men and women, old and young and children who come to the health center.

2. ORS is provided for diarrhea patients.

3. Obstetric patients coming to the hospital are given necessary advice including antenatal checkup and iron

Tablets are provided.

4. Sputum is collected for sputum examination of tuberculosis patients under the National Tuberculosis and Leprosy Control Programme
And free medicines are provided to tuberculosis and leprosy patients.

5. Children and women are vaccinated under the EPI programme.

6. Health, nutrition and reproductive health education is provided to patients coming to the sub-health center.

7. Reproductive health and family planning among adolescents and able-bodied couples attending sub-health centers

Activities are managed.

8. If necessary, the patient is referred to the upazila hospital.
9. Incoming patients and their relatives are concerned for necessary advice and guidance regarding health care
Can easily communicate with doctors.
10. Necessary number of notice boards are installed in sub-health centers in a conspicuous place.
Necessary information is recorded on the notice board.
11. Medicines are provided free of cost from the service center subject to supply. But no need for treatment

A drug may have to be purchased by the recipient from outside the center.

12. List of drugs stocked on board, list of services provided, list of doctors providing services

is pulled
